Title / Ownership Forms

TL-100 Transfer Affidavit

Used to transfer ownership of a manufactured home when a standard title is not available or when an affidavit is required to complete the transfer.

TL-101 Transfer Without Sale Affidavit

Used to transfer ownership without a sale, such as, surrendering a home to the community to cancel liens, gifting a home, or lienholder transfers after repossession.

TL-109 Title Search Request

Used to request a search of title records, including owner information, lienholder details, or current title status.

TL-110 Affidavit of Conversion to Real Property

Used to convert a manufactured home from personal property to real property when it has been permanently installed and recorded with the county.

TL-111 Conversion From Real Property to Personal Property

Used to convert a manufactured home from real property back to personal property, often when the home will be moved or sold separately from the land.

Lien Forms

TL-108a Notice of Lien

Filed by a lienholder to record a legal claim against a manufactured home when money is owed.

TL-108b Notice of Sale by Auction

Used to provide notice of an upcoming auction resulting from an unpaid lien on a manufactured home.

TL-108c Notice of Release of Claim of Lien

Filed to release or remove a previously recorded lien once the dept has been paid.

TL-108d Affidavit of Lien Satisfaction

Certifies that a lien has been fully satisfied. Often submitted with supporting documents.

Supporting Forms / Supplemental Forms

TL-108B Lien Satisfied for Lienholders

Used by lienholders to officially indicate that their lien has been satisfied and may be removed from the title.

TL-103 Title Correction Affidavit

Used to correct errors on a form such as TL-100 Transfer Affidavit to correct names or ownership details.

TL-104 Power of Attorney

Allows an individual to authorize another person to complete title transactions or sign documents on their behalf.

TL-105 Buyer's Acknowledgement of Taxes

Confirms the buyer understands the tax obligations and any outstanding taxes associated with the manufactured home.

TL-106 Affidavit of Entitlement

Used when an individual is legally entitled to ownership of a manufactured home. Only use if it applies to your situation.

TL-107 Affidavit of Dismantling

Used to report that a manufactured home has been dismantled or destroyed so that ownership and tax records can be updated.

TL-112 Bill of Sale

A standardized bill of sale used to document the transfer of a manufactured home between a buyer and a seller.
